A note from the School Nurses:

The summer months are a great opportunity to make any doctor's appointments, get up to date on immunizations, and to have physicians complete medication forms/action plans for next school year.

1. Medications including daily prescription, inhalers, epi pens & diabetic supplies should be picked up during the last week of school. Any medications not picked up by the last day of school will be discarded.

*We can NOT send controlled medications home with students.

2. Medication forms must be updated each year. You will need to complete a NEW “Consent for Prescription Medication Administration” with physician and parent/guardian signatures. Provide this completed form by the first day of school along with an original pharmacy labeled container of their medication and no more than a 30-day supply.

3. Make sure that your child’s health information is up to date. If there are any health concerns please notify the nurse. Additional paperwork is required for life-threatening conditions such as Asthma, Severe Allergies, Diabetes and Seizures. All paperwork must be completed prior to the start of school. Remember, health plans must be updated with each new school year and as needed throughout the year.

4. All immunizations must be current. If your child has medical exemptions they must be updated with each school year unless a letter on file from the physician states a life-long condition. If your child has an exemption affidavit for Immunizations for Reason of Conscience it must be updated every 2 years and must be requested from the Texas Department of State Health Services: https://www.dshs.state.tx.us/immunize/school/exemptions.aspx .

If you have questions, please contact your campus, and we can put you in contact with the nurse.
