Below you will find links to files or forms that you can view, print, fill out, and send back to the school if they apply to your child.

Transcript Requests
You must fill out one of these forms if you are in need of your high school transcript.
*Current High School Students - return this form to high school counselor.
*Former students of GHS - return this form to the administration office.

Absence note - You may fill this in and send to the office. Please remember all notes for excused absences must be received within two days of the absence.

Free and Reduced Meal Application - Please print and fill this out if you feel you are eligible for free or reduced meal charges.

Yearbook baby picture form - Senior parents are asked to fill this out at the beginning of their child's senior year to put in the yearbook. All forms must be received by the end of October to be considered for placement in the yearbook.

High School and Middle School Athletic Forms - Every athlete must have these forms on file with the district. If you have not filled them out, please do so ASAP.
